Why This Matters Now
Virginia communities are facing a moment of real uncertainty. Federal policy changes —including the rollout of H.R. 1 — are projected to shift Medicaid coverage and eligibility for thousands of Virginians in the months ahead. Notices about benefits, coverage, and enrollment requirements are expected to reach households across the Commonwealth this fall and winter, often at the exact moment people are least equipped to navigate them.
Community Health Workers (CHWs) have always been the trusted bridge between healthcare systems and the neighbors they serve. CHW Connector puts that trust on wheels — a mobile outreach vehicle staffed by CHWs, doulas, and health navigators that travels directly into communities to make sure no one falls through the cracks of a changing system.
What CHW Connector Does
The CHW Connector van travels on a rotating schedule across Virginia's health regions, bringing:
General outreach & navigation
help understanding health coverage, benefits, and social services, with a focus on the social determinants of health that shape everyday wellbeing.
Maternal health support
a doula and maternal health provider ride alongside CHWs to reach communities in maternity care deserts.
Outreach to unhoused and unstably housed neighbors
meeting people where they are with connection to services and resources
Each stop is designed around what the community actually needs — not a one-size-fits-all
checklist.
Where We're Headed: The Statewide Rotation
CHW Connector is built to reach every region of the Commonwealth on a recurring cycle, so no part of Virginia is left waiting:

Within each regional visit, the van follows a four-week rhythm: two weeks of general outreach and navigation, one week centered on maternal health, and one week dedicated to unhoused outreach — so every community gets the full range of support over time, not just a single pass-through.
